Excursion & asthma, Anaphylaxis & allergy forms

Many parents download the Excursion Form and relevant Asthma/Anaphylaxis/Allergy Form to their computer, complete the details and keep it on file. This is usually quicker and more convenient than completing the form from scratch for each excursion.

Please note that all students with a diagnosed allergy must have the appropriate form completed, including parents' emergency contact details and a colour photo of the student, and then signed by their doctor. This form must be forwarded to the School Health Centre, where it will be kept on file. A copy must also be included with each excursion/camp notice.

All members of our community - past, present and future - were invited to join us as we celebrated fifty years of outstanding education on the Mornington Peninsula. We had a host of celebratory Golden Jubilee events in 2011.
